Herniated Disc

The endoscopic removal of a hernia is a cohesive, percutaneous operative treatment. Ectopic or leaked-out tissue is removed, thus leaving the empinched nerve freed.

Severe disc-degeneration causes constant, sometimes immobilizing back-pain. Typical symptoms of degeneration are strong, chronic back-pain and paralysis of the legs. Many patients with degeneration have had numerous hernias previously.
Alternatives to fusion:
Most clinics still prefer fusion-operations for chronic back ailments. In our opinion a fusion should only be considered, when alternatives like abrasion or artificial disc have failed or do not promise a good outcome.
Artificial Disc
Artificial disc- surgery is conducted over the abdominal approach. The degenerated disc is removed during this operation and replaced by a polyethylene core between two titanium endplates. The prosthesis is fixed between the two vertebral bodies and remains stable, enabling normal flexibility between the vertebral bodies.
Abrasion
An abrasion or discoplasty is recommended if, in addition, a distinctive degeneration of the disc is diagnosed. The minimally invasive abrasion is not a prosthetic- or fusion-procedure. The damaged or degenerated tissue of the nucleus is removed, thus leaving the outer ring intact. The disc is approached from two sides through a small incision.
Spondylodesis = Spinal Fusion
All other alternatives for the treatment of chronic back-pain – like the endoscopic nucleotomy, abrasion or disc-prosthesis - are considered, prior to recommending a spinal fusion. But certain spinal conditions can only be mended by means of a spinal fusion. Particularly for patients with a spondylolisthesis or after several spine operations, the spinal fusion often means the last resort.
Sliding Disc
In case of a slipped disc and a stenosis of the spinal canal, whereas the slipped disc only slightly disarranges (proven by means of motion X-ray) only the stenosis is treated microscopically.
In case of an in-stabile slipped disc with a stenosis a microscopic discectomy in connection with a ligament is performed.
